MRI features (1a,d) T2WI (axial and coronal) with bilateral, extensive, diffuse symmetrical hyperintensity (similar to CSF) in white matter involving deep and subcortical white matter, arcuate fibres and corpus callosum. Thalami are atrophied bilaterally (1d: arrow). 1b) FLAIR (axial): The cystic white matter has a low signal like CSF. 1c) T1WI (axial) revealing hypointensity in the same regions as above. Preserved deeper nuclei has slightly hyperintense signal. 1e) MR spectroscopy of white matter showed severe decrease in NAA, and a lipid-lactate peak.
